IP Lookup talks to ipwho.is directly. When an IP is entered, the request path contains that address; when the field is left blank, the service root is requested and the third party identifies your public exit IP from this network connection. Localisable fields such as country, region, city and continent are requested in the current page language; where the source only ships generic Chinese, the page converts the value for display. The result can also include ISP, organisation, ASN, time zone and latitude/longitude inferred from the database.

Yes, and ipwho.is can necessarily see the public exit IP of the request from the HTTP connection.
The IP you enter is sent to ipwho.is; when the field is blank the third party still sees your exit IP. Clicking the map link also reaches OpenStreetMap. Use this only for lawful network diagnostics, for assets you manage, or for public information; do not treat rough IP geolocation as a home address or proof of identity, and do not use it for harassment, tracking or unauthorised reconnaissance.
